EHRA Congress 2026 – Prof Roland Tilz (University Hospital Schleswig-Holstein, Lübeck, DE) presents findings from the LEXIS Registry, a large-scale, multinational retrospective observational study examining whether simultaneous lead extraction and leadless pacemaker implantation represents a safe and effective alternative to the traditional delayed re-implantation strategy in patients with pacemaker infection.
The registry enrolled over 500 patients across 28 centres in eight countries, encompassing a high-risk population with both pocket and systemic infections — providing robust real-world data on a clinically significant and resource-intensive patient group.
Findings demonstrated no significant difference in reinfection rates, 12-month mortality, or device upgrade rates between the simultaneous and delayed re-implantation approaches, while the simultaneous strategy was associated with earlier hospital discharge and the opportunity for home-based IV antibiotic therapy — with potential benefits for both patients and healthcare systems.
